With 4-page booklet. Some copies have barcode sticker on cd case. Some copies may also have Dick's Picks 1-18 catalog.
The first disc on some copies had a manufacturing defect causing a 'pop' during 'Queen Jane' at approx. 1 minute 10 ten seconds into the tune. Replacement discs were enclosed with orders from the Grateful Dead Mercantile.
